National Record Claim Form - All records are subject to verification. Record Claims sent to the USA Archery office for consideration MUST have a completed "NAA Record Claim Form" and a legible copy of the archer's scorecard. The scorecard MUST have been signed by a recognized tournament official (preferably the director of shooting or else the head judge, or both).
